Bumps effect and the @effect/* dev packages from 4.0.0 to 4.0.6, and the peer range to ^4.0.6. This comes first so the roadmap PRs in #52 can be reassessed against what 4.0.6 already ships: SqlClient.BeginTransaction (#72 / #101) and SqlClient.afterCommit (#73 / #100).
What the bump changed
A failed COMMIT now fails, not dies. 4.0.6 puts the COMMIT's SqlError in the error channel. It is still a die when the cleanup after it fails too. restoreCause now maps a SqlError failure after a successful body to TransactionCommitFailed, as it already did for the defect. ROLLBACK failures still die and still replace the body's exit, so that path is unchanged.
Effect's statement span is named after the database. It takes db.namespace, falling back to the server address, then db.system.name, instead of sql.execute. The span tests now find it by its effect.sql.method attribute, and the ClickHouse HTTP test asserts the new name.
The @effect/vitest Bun patch is ported to 4.0.6. Upstream still reads V.TestRunner eagerly and still assumes ctx.signal.
Docs that pinned 4.0.0 now say 4.0.6. A dated note in design/transactions.md records the COMMIT change.

Bug Fixes
Failed transaction commits are now reported as typed SQL errors instead of terminating unexpectedly. Rollback failures continue to be reported as defects.
Improved compatibility with Bun’s test context.
Documentation
Updated installation instructions and examples to Effect 4.0.6.
Clarified how commit and rollback failures are reported.
Tracing
Database statement spans now use the database name, and ClickHouse HTTP connections report their driver name.
Bump effect and the @effect/* dev packages from 4.0.0 to 4.0.6, and the
peer range to ^4.0.6.
- 4.0.6 fails a transaction with the COMMIT's SqlError instead of dying.
restoreCause now treats a SqlError failure after a successful body as
TransactionCommitFailed, as it did for the defect.
- SqlClient.make requires `driver`; ClickhouseHttpClient passes
"clickhouse-http".
- Effect names its statement span after the database, not sql.execute;
the span tests find it by its effect.sql.method attribute.
- The @effect/vitest Bun patch is ported to 4.0.6.

Effect dependencies and installation guidance move to 4.0.6. The Vitest patch resolves the current suite at call time and calls settleOnAbort only when a context has a signal.
When the transaction body succeeds, restoreCause now recognizes SqlError in a failure or defect and returns TransactionCommitFailed. The documentation distinguishes COMMIT failure from ROLLBACK failure.
Driver and tracing updates src/clickhouse-http/client.ts, src/clickhouse-http/client.test.ts, src/database/tracing.ts, src/database/tracing.test.ts
The ClickHouse HTTP client identifies its driver as clickhouse-http. Tracing tests locate spans by attributes and expect the database span name.
MapleTechLabs/effect-orm#77 — The PR changes statement span naming expectations and tests to find spans through the effect.sql.method attribute. It does not implement the full request described by the issue.

# Migrating from Effect v3 to Effect v4
> Note: If you run into a migration issue this guide doesn't cover, let us know on Discord or open an issue.
## Background
Effect v4 is a major release with structural and organizational changes across the ecosystem. The core programming model — `Effect`, `Layer`, `Schema`, `Stream`, etc. — remains the same, but how packages are organized, versioned, and imported has changed significantly.
### Versioning
All Effect ecosystem packages now share a single version number and are released together. In v3, packages were versioned independently (e.g. `effect@3.x`, `@effect/platform@0.x`, `@effect/sql@0.x`), making compatibility between packages difficult to track. In v4, if you use `effect@4.0.0`, the matching SQL package is `@effect/sql-pg@4.0.0`.
### Package Consolidation
Many previously separate packages have been merged into the core `effect` package. Functionality from `@effect/platform`, `@effect/rpc`, `@effect/cluster`, and others now lives directly in `effect`.
Packages that remain separate are platform-specific, provider-specific, or technology-specific:
* `@effect/platform-*` — platform packages
* `@effect/sql-*` — SQL driver packages
* `@effect/ai-*` — AI provider packages
* `@effect/opentelemetry` — OpenTelemetry integration
* `@effect/atom-*` — framework-specific atom bindings
* `@effect/vitest` — Vitest testing utilities
These packages must be bumped to matching v4 versions alongside `effect`.
### Unstable Module System
v4 includes unstable modules under `effect/*` import paths. Their paths no longer contain an `unstable` segment, but their API documentation is marked with `@stability unstable`.
`@stability unstable` means an API may receive breaking changes in minor releases. `@stability experimental` means it may receive breaking changes across patch versions. `@stability stable` and APIs without a stability tag follow strict semver. Every module and directly importable export declares its stability explicitly. Modules in packages other than `effect` are currently `@stability unstable`.

## Summary
When `BEGIN` itself fails inside `withTransaction` (for example `SQLITE_BUSY` under `BEGIN IMMEDIATE` after `busy_timeout` expires), `makeWithTransaction` in `effect/unstable/sql/SqlClient` still issues `ROLLBACK` in its failure branch. No transaction is active at that point, so the `ROLLBACK` fails, and because it is wrapped in `Effect.orDie` the original typed, retryable `SqlError` is discarded and replaced by a defect. Callers can no longer classify the error as busy/locked and retry.
The failure branch does not distinguish "the wrapped effect failed" from "`BEGIN` failed":
// effect/dist/unstable/sql/SqlClient.js (makeWithTransaction)
(id === 0 ? options.begin(conn) : options.savepoint(conn, id)).pipe(
Effect.flatMap(() => /* wrapped effect */),
Effect.exit, // <- captures a BEGIN failure too
Effect.flatMap((exit) => {
// failure branch: runs even when BEGIN never opened a transaction
effect = Effect.orDie(id > 0 ? options.rollbackSavepoint(conn, id) : options.rollback(conn))
...
## Why rc.108 exposes it
The `makeWithTransaction` code is byte-identical to `4.0.0-beta.102`, but the branch was unreachable there: the beta.102 sqlite client set neither `beginTransaction` nor `busy_timeout`, so plain deferred `BEGIN` acquired no lock and never failed. Contention surfaced on the first statement inside an already-active transaction, where `ROLLBACK` is valid and the typed error propagated. The rc.108 sqlite client sets `beginTransaction: "BEGIN IMMEDIATE"` and `PRAGMA busy_timeout` for writable connections (`SqliteClient.js:199` and `:66-67`), so `BEGIN` itself can now fail under contention and reach the latent branch.
effect/sql/SqlError: Failed to execute statement
effect/sql/SqlError/UnknownError: Failed to execute statement
Error: cannot rollback - no transaction is active
`
## Suggested fix
Only issue `ROLLBACK` when `BEGIN` succeeded (track begin success, or keep `begin` outside the `Effect.exit` region), and let a `BEGIN` failure propagate as its original typed `SqlError`.
## Prior art check
Closest existing issue is #3709 ("Transactions in sql-libsql do not work", `cannot commit - no transaction is active`) — closed, different symptom. I found no open issue for this failure.
## Downstream impact
Concurrent-writer contract tests (two writers read-modify-write one row; retry on busy) fail deterministically: the busy error arrives as a defect, so busy-classification retry logic never sees it.

Preserve cleanup defects when COMMIT also fails.
Effect 4.0.6 closes the connection scope with the transaction exit. If COMMIT fails and scope cleanup adds a defect, return the original cause instead of replacing it with TransactionCommitFailed. The current branch selects one SqlError and discards the remaining cleanup defect.
