Fixes large HTTP/2 downloads over slow links failing with
{stream_error, internal_error}and pooled TLS connections being logged as killed on close (#976). Updates h2 to 0.12.5 and certifi to 2.18.0, which drops the compromised eMudhra certificates (#975).
