feat(ux): one Activity log — changes & sign-in | tool calls - #225

UX phase 1 (docs/plans/ux-simplification.md). Evidence (tool-call trail) and Audit log (settings, members, invitations, sign-in) were two Governance rail items with near-identical names and the same question behind them: "what happened here?". They are one entry — Activity log — with two views; each view is the existing component (data access, CSV export, role checks unchanged). The old evidence deep link / search hit opens the Tool calls view with Activity log lit; the standalone Audit log and Evidence apps stay launchable from Activities.
