Editorial pass so we can publish a new working group draft. Last published text was Implementer's Draft 1 in 2019. This takes the working copy to draft 04 and adds a changelog.

The open CIBA PRs (#865 unsigned requests, #866 binding messages, #867 inherit TLS/algorithms) should land before we actually publish, they are not in this PR.
