FTUE: pull request template (#32)

Summary

Adds the default GitHub pull request template at .github/pull_request_template.md.

The template asks for a summary, a related issue (Closes / Fixes), and how to verify. The checklist covers recorded evidence, green CI, no secrets in the pull request body, human CODEOWNERS review and merge (authors do not self-merge), and public-safe wording. It links to CONTRIBUTING.md, .github/CODEOWNERS, and the verify, no-self-merge, and human-gate principles.

How to verify

• The diff adds only .github/pull_request_template.md.
• Relative links in that file resolve (the validate workflow checks this).
• GitHub uses .github/pull_request_template.md on the default branch as the pull request body. After this merges, open a new pull request and confirm the template is prefilled.

Checklist

• Evidence is recorded above (summary and how to verify).
• CI is green (required checks on this pull request, including validate).
• No secrets in this pull request body or diff.
• A human in CODEOWNERS reviews and merges. Authors do not self-merge.
